Office policies

• We request that all patients submit a copy of a valid picture Identification Card and a copy of their insurance card during their initial visit.
• Please be prepared to submit the Social Security number and Date of Birth of the injured patient during their initial visit.
• If X-Rays, MRI Cat-Scan or other radiographic studies were taken of the injured area, it is the responsibility of the patient to obtain the actual FILMS or DISC. We will require that the patient bring the films or disc to the office visit. On certain occasions appointments will need to be re-scheduled if the actual radiographic films are not available for the doctor to view.
• All co-pays are due on the date of the visit.

Insurance

• As a courtesy, we bill your insurance for treatments rendered.
• We accept all Workers' Compensation Insurances.
• Please call our office for a list of our PPO Insurance acceptance.
• Cash Patients are Welcome/HMO
